3) Dental consent form
4) Media consent form
5) Disclosure of student directory information
6) Request for emergency and health information
In addition, please bring in $15 for the workbooks and $35 for fine arts.
The sooner we get these forms checked in, the sooner we quicker we can dive into classwork in the mornings.
